Model of Care

We are a Recovery based service and incorporate elements of the Good Lives Model, supported by individualised interventions. We have a full multidisciplinary team (MDT) with Nursing, Consultant Psychiatrist, Psychologist, Occupational Therapist and Social Worker support. Given the specific needs of our Service Users, the MDT work on an interdisciplinary basis, informed by functional analysis to develop highly personalised care plans, focussed heavily on emotional regulation, increased self-esteem and the development of anxiety management skills. All Service Users participate in a robust therapeutic programme which includes community involvement, education at local colleges, a focus on physical healthcare, support with day-to-day activities and life skills as well as daily planning meetings. This enables individuals to participate in therapy and be fully engaged in their treatment and recovery.

 

Admission criteria

  • Adults 18 years of age and upwards
  • Detained under the Mental Health Act 1983 (amended 2007)
  • Have complex mental health needs
  • Display behaviours that challenge

 

Environment

Victoria Gardens is a modern purpose built facility which provides small wards with en-suite bedrooms. Each ward has its own dining area, communal lounge, recreational area, laundry and a rehabilitation kitchen which provides Service Users with the opportunity to develop their daily living skills with the support of the nursing team and Occupational Therapists. There is also separate therapy space.

There are four self-contained apartments which provides a step through pathway from inpatient stay to pre discharge to residential community living. The apartments have their own front door and provide a bedroom, kitchen/ lounge and private bathroom. Service Users are encouraged to self-cater, manage their medication and engage with the many real work opportunities available within the local community.

Wi-Fi is available throughout the building and Service Users have access to technology as a learning, social and leisure resource.

A dedicated Skype room is also available for use along with a multi faith room.

 

Our outcomes aims:

  • Decreased dependency on in-patient facilities through the use of dynamic risk assessment and risk management strategies
  • Increased self-worth and social inclusion
  • Maximised access to everyday community facilities
  • Management of positive and negative symptoms of mental illness through effective prescribing of medication, psycho-social support, increased motivation and engagement
  • Ensure the early identification of signs of impending relapse and the use of proactive strategies to avert or significantly reduce the severity of relapse and avoid whenever possible any subsequent reduction in social functioning
  • Adherence and co-operation with treatment and increased insight into mental health needs
  • Collaborative working with families involving them with the Service User’s consent and agreement in care assessment, planning, implementation and evaluation in the future provision of care
  • Proactively maintain and improve the individual’s physical health, increase healthier living, diet and exercise and provide regular and timely access to their GP, Dentist and all relevant health screening programmes
  • Minimise the prevalence and degree of uncomfortable and sometimes harmful side effects of medication through regular monitoring using LUNSERS and other evidence based assessments
