Ty Grosvenor is a rehabilitation service for men and women with a Mental Illness and/or Personality Disorder. The services have of two distinct wards and two, two-bedroom apartments that support independent living. The ethos of the service is Work, Rest and Play and focuses on equipping Service Users with the appropriate skills for community living.
There is significant emphasis upon accessing mainstream resources within the local community, providing patients with opportunity to develop robust community skill. The service is located in Wrexham, North Wales and just a short walk away from the town centre, cafes and restaurant, leisure facilities and local colleges. Using community resource will assist patients in generating social networks external to the service that can be utilised upon discharge from the service, mitigating potential relapse or isolation.
The WISHE Model
There is a culture of empowerment and a strong emphasis on ownership and responsibility to support recovery at Ty Grosvenor. The model of care at Ty Grosvenor is supported by our WISHE Care Pathway which puts the Service User right at the heart of their care and treatment. WISHE incorporates five core zones that together form a bespoke pathway chosen by the Service User, the family and the care team and is tailored to individual need. The core zones are responsive to change and comprise of Work, Interventions, Social Networks, Health and Empowerment. A positive experience is critical to the recovery and rehabilitation pathway and our model promotes collaboration, innovation and empowerment.
