Specialist apartment based model of care for women with complex mental health needs
More information about Crossley Place
Crossley Place provides a new and innovative approach to providing care and quality of life for women with complex mental health needs.
The service offers a positive alternative for women to be managed outside a conventional hospital setting, working within the principles of least restrictive practice and positive behavioural care plans. The desire to provide a choice is key to making a real difference to patient’s lives.
The service is aimed at individuals who challenge mainstream services which traditionally offer a more hospital or ward based approach, with little or no alternatives to this. The single apartment model and clinical approach places the patient at the heart of all we do and has a real influence on how their care and treatment is driven.
Model of Care
The service has two single occupancy apartments, where individuals are supported by a specialist nursing team who are trained in Positive Behaviour Support (PBS), Autism, Dialectic Behaviour Therapy (DBT), Eye Movement Desensitisation and Reprocessing (EMDR) and other specialist approaches. Each team is led by a Registered Nurse supported by the Registered Manager. Recovery Workers work alongside Nurses to support the delivery of care 24 hours a day.
We build packages of care based on individual need where everyone has access to the Responsible Clinician, Psychologist, Occupational Therapist, Social Worker and other members of the Multidisciplinary Team (MDT) as required.
Outcome & Benefits
We offer a holistic approach to delivering care that is individually agreed and not influenced by existing institutional timetables. Whilst recognising the need for routine, the care reflects what the individual in partnership with the MDT believe will be of benefit. We have regular reviews of care and treatment in Individual Care Reviews (ICRs) and these take place, as a minimum, every two weeks or as agreed by the MDT.
The service is aimed at women with complex mental health needs who may challenge service and for who the current models do not meet their needs. All referrals are assessed by the local team and a decision made on service delivery and compatibility with the other service users.
We would expect each individual:
Community Integration & Discharge
The nature of the service means it is not a short stay facility but will in time, allow residents to return to the community. Clear outcomes and discharge planning are agreed prior to women coming to Crossley Place.
The service compromises of individual apartments with access to shared living environments and therapy space. The apartments are spacious and allow people to have their own individual space with staff to support 24 hours a day.
This is a model based on each individual’s right to make individual choices about their lives and care, in an environment that manages risk positively with the least restrictive approach.
