Complex Care Services
Our complex care services are designed to support individuals with a learning disability and/or autism with highly complex and severe behaviour which challenges services and associated risks.
Complex care services are developed in partnership with a Local Authority and or Clinical Commissioning Group to meet an identified local demand and with specific individual’s in mind. These services are for individuals from that specific area to enable them to be repatriated closer to home.
The environment is a unique and important part of the model. Care is provided in single person apartments. This allows people to live in their own space for care and support to be delivered in a domestic environment which replicates community living more closely than a traditional group living setting. The apartments also allow bespoke care to be delivered for the individual and risks and behaviours to be more effectively managed. Apartments are designed around the individual with high specification, robust furniture, fixtures and fittings.

Our complex care services are designed as:
- Step down services for those who are currently or have previously been in hospital and require a robust environment and package of support to progress to a more independent living setting. They are also for those or who need a high level of support to live within the community and will continue to require this longer term.
- Step up services where an individual is living in the community and due to crisis requires a period of person centred intensive care and support in a community-based service to avoid an inappropriate hospital admission.
The aim of these services is to allow individuals to live independently but with a high level of support, through a programme of person- centred care which incorporates the following elements:

- Individually tailored care and support delivered though high intensity staffing packages

- Strengths based approach
- Meaningful and stimulating activities and opportunities
- Enabling independent living skills and community engagement
- A strong emphasis on safeguarding
- Solid clinical governance

- Strong multiagency and family partnerships to ensure a holistic package of care and pathway
